TERMS AND LIMITATIONS OF THE PLAN:

• Family members cannot be substituted in for another family member. Children must be under the age of 18 and a full-time student. All family members must live in the same household.

• Cleanings must be completed before the coverage date ends.

• Patient must be an active plan participant at the time procedures is completed in order to receive the additional discount.

• This is a dental discount plan and is NOT dental insurance. This plan cannot be combined with dental insurance.

• The plan discounts do not apply to any treatment covered by insurance such as: Medical, automobile, homeowner’s, Medicare, Medicaid, worker’s comp, Veterans Administration, etc.

• Payment is due at the time of service unless other payment arrangements have been made.

• Annual enrollment fee is due at the time of enrollment.

• Rates are subject to change annually.

• Must remain a plan participant during entire duration of Smart Moves Orthodontic treatment.

• Plan dues are not refundable or transferable. Even if you fail to use your free benefits. Benefits do not accumulate. Use it or lose it.

• Plan is good only at Lighthouse Dental Group; therefore if you are referred to a Specialist not affiliated with Lighthouse Dental Group, discounts offered to plan participants will NOT apply.

• If you reserve time for a benefit and fail to show for your reserved time then you forfeit that benefit.

• THIS IS AN ANNUAL PLAN. ALL BENEFITS MUST BE USED WITHIN THE DATE OF THE PLAN. PLAN BEGINS THE DATE THE PLAN FEE IS PAID AND ENDS ONE YEAR LATER.
